Not sure which forms you’ll need?  Keep reading below to see which ones to fill out before your first visit.

 Explanation of Forms

There are some forms that need to be filled out in order for us to work with you.  You can print these from the website, fill them out and bring them in with you. Or, you can do it at the office. Let us know your preference when you set up your meeting and we will schedule accordingly.

Please wait to sign any forms that need a witness signature until you meet with your therapist and they will sign as witness.

  1. Consent to Treat: Central to effective and ethical therapy is informed consent. The Consent to Treat form on this website give the general outlines of the therapeutic process, what you can expect or may encounter as therapy progresses, and what the general policies and procedures followed in this office. You are welcome to read and fill it out online, and we will review it at your first session so that we can answer any questions you have.

  2. Fee and Insurance: If you have insurance and want to fill this out ahead of time, please feel free to do so. A copy of your insurance card will be made when you come to your appointment, but having the fee agreement filled out will save some of your time. For sliding fee scale arrangements, this is decided on a case by case and there is no form to fill out in advance.

  3. Notice of Privacy Practices: People are entitled to know and understand their rights to privacy, and the limitations of that right. This form gives you that information and an explanation as to the how’s and why’s of disclosure or protected health information. You can sign this in advance and bring to the office with you.

  4. Release of Information: Except in strictly limited situations as provided by law, none of your information can be shared with anyone without your consent. If you have family or medical or other professionals you want us to be able to share your information with, please fill out an authorization for each person and bring to the office with you for your first appointment.
